Motor Vehicle Accidents
- Accident reports can be obtained in person or by mail.
- To receive an accident report by mail, send a check or money order for $6.00 with the report number to the following address:

Please allow 10 working days for your report to be processed. For accidents that did not happen within the city limits of Maypearl, please call the Texas Dept. of Public Safety at 972 923-6780.
The “Blue Form” is for Drivers of a motor vehicle involved in a crash not investigated by a law enforcement officer and resulting in injury to or death of any person, or damage to the property of any one person, including himself, to any apparent extent of at least one thousand dollars ($1,000), must within 10 days after such crash complete and mail it to the State.
Accident reports completed by law enforcement officers in Texas are all reported on the same form and use the same codes regardless of what agency filed the accident report.

Texas Driver's Crash Report (Form CR-2)
This form is required for drivers involved in a crash that was not investigated by a law enforcement officer. You must complete and send this report to TxDOT within 10 days if the crash resulted in injury, death, or at least $1,000 in damage to any person’s property, including your own. The form includes sections for crash location, vehicles involved, injuries, and a brief statement from the driver.
